The PAWS4POTTER event “Making A Mark On K9 Cancer” held on 10/21/17 at Thompson Park of Watertown, NY, is a dog-walking event to benefit pet cancer awareness via Paws4Potter and to benefit the National Canine Cancer Foundation (a 501c-3 non-profit organization).
As this is a dog-walking event, I (as a participant) understand that there may be associated risks to me or to my dog(s) by participating in this event, including but not limited to falling, interaction with other participants, dog-dog interaction, effects from the weather, and conditions of the park walkways. I hereby assume all risks, including personal injury to me or to my dog(s), property loss, and other damages that may be incurred by participating in this event.
It is my sole responsibility to dress appropriately for this event. I am solely responsible for my own health and safety, as well as the health and safety of my dog(s). I am physically fit and able to participate in this walking event. If walking with my dog(s), I represent that he/she/they are physically fit and able to participate as well and that he/she/they are current on Rabies vaccination and are not in heat at the time of the event.
I understand that PAWS4POTTER and Thompson Park are not responsible for any of the above described risks that may be incurred by my participation (or the participation of my dog(s)) at this event.
I grant permission to the organizers of this event to use my and/or my dog’s name and image as an event participant in photographs and videos.
I have read and understood this waiver and agree to its terms.
If the participant is under the age of 18 years, I – as the parent or legal guardian – hereby consent to his/her participation in this event and agree, on behalf of myself and the participant, to the terms of this waiver.
